Intermittent misfires at warm idle are often harness/connector related. Inspect pins for fretting, check resistance, and verify trims in the ECM.
Loss of power or high EGTs? Pressure test the CAC and boots. Even small leaks tank efficiency.
Soot trails near joints or warped flanges are common with age. Replace gaskets/studs; torque in sequence after heat cycles.
Sticky valves and faulty delta-P sensors cause rough running and regen issues. Confirm sensor plausibility and commanded vs. actual EGR flow.
Front cover and crank seals age out. Verify with dye and a clean engine before install to avoid chasing false positives.
